ANALISIS IMPLEMENTASI DISCHARGE PLANNING TERINTEGRASI DI RUANG RAWAT INAP RSUD MAJENE: STUDI KUALITATIF

Background: Implementing integrated discharge planning can speed up recovery, reduce length of stay and reduce patient anxiety by providing clear information regarding home care.
Objective: To explore PPA's experience in implementing integrated discharge planning in the inpatient room at Majene Hospital.
Method: Research uses a qualitative design. The number of informants was 7 heads of rooms. Data were collected through semi-structured interviews in August 2025 using an interview guide on integrated discharge planning.
Results: Implementation of integrated discharge planning at Majene Regional Hospital has been running in most of the inpatient rooms. The discharge planning process focuses more on the final stages of treatment and does not start from the moment the patient enters the hospital. Documentation is not yet uniform and interprofessional collaboration is limited in implementing discharge planning. The main obstacles include the absence of families during education, administrative and digital system obstacles, and lack of training.
Conclusion: Increased coordination between PPA, involving families and training of health workers is needed to realize effective and sustainable discharge planning.
